Summer Camp Paperwork

Below are links to the paperwork that we need on file for every camper to attend camp.  Everyone should have received a confirmation packet that contained this paperwork.  If you did not, please call our office so that we can confirm your address.

Needed paperwork

1. Acknowledgement and Consent - must be signed and returned

2. Health History - must be completed, signed and returned (don't forget to attach a photo and copy of your immunization record and insurance card.)

3. Over the Counter Meds - this form tells us if your child is allergic to or should not take specific over-the-counter medications that we may use at camp to treat common ailments.

Other helpful forms

1. E-mail Card - This is kept in the e-mail cafe to help your child with addressing e-mails to the correct address.  It is not needed for campers who know their parents e-mail address by heart.

2. Transportation Request - If you would like us to provide transportation to and / or from the St. Louis International Airport, than you must return this form.

3. Final Payment Form - Helps you calculate your final payment.

4. Course Request - This form is for the camper to complete so that we know what activities everyone is wanting to do at camp.
