You have reached our summer paperwork page.  Be sure to read the parent handbook.  The forms will need to be printed off, completed and returned to our office by April 1.

Below are links to the paperwork that we need on file for every camper to attend camp. 

Parent Handbook - Please read this!  This is our handy book that was created to answer all you questions about camp. - It's really for campers too!  In this you will find a packing list, directions to camp, drop off and pick up times and your pass code for accessing this summer photos and newsletters.

Needed paperwork

1. Health and Medication Form - must be completed, signed and returned (don't forget to attach a photo and copy of your immunization record and insurance card.)

2. Authorization and Consent Form

Other helpful forms

1. E-mail Card - This is kept in the e-mail cafe to help your child with addressing e-mails to the correct address.  It is not needed for campers who know their parents e-mail address by heart.

2. Transportation Request- If you would like us to provide transportation to and / or from the St. Louis International Airport, than you must return this form.

3. Final Payment Form - Helps you calculate your final payment.

4. Course Request - This form is for the camper to complete so that we know what activities everyone is wanting to do at camp.

5. Packing list - your packing list can be found on the second to the last page of your parent handbook.
